This notice does not replace our Notice of HIPAA Privacy Practices, which governs any Protected Health Information held by a dispensing pharmacy.
Through these forms we collect the information you choose to provide, which may include your experience with a treatment, how you are feeling, and how it fits your routine; side effects you have or have not noticed; your history with hair loss treatments, both past and current; and optional details such as age range, sex, and type of hair loss.
You may also choose to provide optional contact information, such as a name or email address, so that we can follow up with you. You can complete the form without giving your name or email. All identifying fields are optional.
We use your responses to support you and respond to questions or concerns you raise; to understand how treatments perform in real life and improve our products and guidance; to compile aggregated and de-identified insights about treatment experiences; and, where you give permission, to share your experience publicly as described below.
A core purpose of these forms is to learn from real treatment experiences and share what we learn. We may publish and share survey results publicly, including on our website, in marketing and advertising, on social media, in research or community summaries, and with the broader hair loss community, in pseudonymized form.
Before sharing publicly, we remove information that directly identifies you, including your name, email address, and order details. Published material may include non-identifying details such as your age range, sex, type of hair loss, dose, how long you have used the treatment, the side effects you reported, your prior and current treatments, and your own words about your experience.
We do not publish your name. Anything we share publicly is pseudonymized, and we will not publish your name, email, photographs, or other directly identifying information.
Please note that even with direct identifiers removed, a combination of details could in rare cases make a response identifiable to someone who already knows you. If you would prefer we not share your individual response publicly in any form, select “No” on the marketing question in the form, or email privacy@anagen.xyz, and we will honor your choice.
We only use your individual check-in response in marketing or testimonials if you tell us we may, through the consent question in the form. You can withdraw that permission at any time by contacting us at privacy@anagen.xyz, and we will stop using your response in new materials going forward.
Treatment check-in forms and any published results are for informational purposes and are not medical advice. They do not replace guidance from your prescriber or pharmacist. If you have an urgent concern about a side effect, do not wait on the form: reply to our email or contact your healthcare provider. If your treatment is dispensed by a pharmacy, information held by that pharmacy is governed by its own notices, including any applicable HIPAA Notice.
We use reasonable technical and organizational measures to protect check-in responses, and we limit access to people who need it. We keep responses for as long as needed for the purposes described above, after which we delete them or further de-identify them.
